This is not health insurance

This agreement is not health insurance and the health care provider will not file any claims against the patient's health insurance policy or plan for reimbursement of any health care services covered by the agreement. This agreement does not qualify as minimum essential coverage to satisfy the individual shared responsibility provision of the Patient Protection and Affordable Care Act, 26 U.S.C. s. 5000A. This agreement is not workers' compensation insurance and does not replace an employer's obligations under chapter 440.

In short: this is a primary-care membership, not insurance - keep your own coverage for emergencies, hospital stays, specialists, imaging, surgery, and prescriptions. Emergency? Call 911.

Who this is with

This Membership Agreement is between Precision Total Health & Wellness (101 E. Mahoney St, Suite 11, Plant City, FL 33563, (813) 808-2054) and the patient or responsible party who signs below. Precision is a nurse-owned, cash-pay direct primary care practice and does not bill insurance. Your care is provided by Heather Ledford, RN and Sandy George, APRN-BC, a Florida autonomous nurse practitioner.

What your membership includes

Within your provider's license, training, availability, and clinical judgment, your membership includes:

  • Unlimited primary care visits
  • Sick visits when you need them
  • Annual wellness exam
  • Chronic disease management and monitoring
  • Telehealth visits when appropriate
  • Same or next-day visits when available
  • Direct provider communication during business hours
  • Medication management
  • Discounted cash-pay lab work through Quest
  • Member preferred pricing on wellness services

About access

"Unlimited" means no per-visit fee for medically appropriate included visits; scheduling and provider availability still apply. Direct access and telehealth are not emergency services and are not 24/7 coverage. If you are having an emergency, call 911.

What it does not include

Emergency or hospital care, specialists, surgery, imaging, prescription drug costs, supplements, outside lab charges, medical devices, or anything not listed above. We may offer some of these at a separate cash price, and will only charge you for them with your separate approval.

Your fee and how billing works

Choose one: Monthly at $79/mo, or Annual at $849/yr (one payment for 12 months). Billing is handled securely through Stripe; we do not store your full card number. By signing, you authorize us and Stripe to keep your payment method on file and charge it for your membership fee on a recurring basis, plus any amount you separately authorize here. Monthly fees charge on your start date and the same day each month; the annual plan is a single prepaid payment for 12 months and does not auto-renew.

Auto-renewal

Your monthly membership renews automatically, month to month, until you cancel as described below (the annual plan is prepaid and does not auto-renew). We give at least 30 days' notice before any price increase, and a price increase will not change the 3-month starter price already agreed for a current monthly member.

Your 3-month starter commitment (monthly plan)

When you enroll on the monthly plan, you are purchasing an initial 3-month membership package totaling $237, which we let you pay in three $79 installments for convenience. You may cancel at any time. But if you cancel on the monthly plan before all three installments are paid, the remaining unpaid installments become due, and you authorize us to charge them to your card on file through Stripe as a single one-time charge. This is not a cancellation penalty - it is the unpaid balance of the 3-month package you already chose. Cancel after 1 payment: $158 remaining (2 installments). Cancel after 2 payments: $79 remaining (1 installment). Cancel after 3 payments: no starter charge. Annual members prepay the full year, so the starter commitment is already satisfied and no early-termination charge applies.

How to cancel

You may cancel at any time, and either you or the Practice may end this agreement, on at least 30 days' written notice. To cancel, use the online cancellation option in your member portal or email hledford@precisiontotalhealthandwellness.com. Monthly members past the 3-month starter: cancellation takes effect 30 days after we receive notice; monthly fees are not prorated. Monthly members still inside the 3-month starter: cancellation is effective on 30 days' notice, and any unpaid balance of the starter package is charged as described above. The Practice may end this agreement immediately for nonpayment, repeated failed payments, fraud, or conduct that makes the care relationship unsafe or not clinically appropriate.

Refunds

Membership fees pay for access, not per visit, and are generally nonrefundable once charged, except for a duplicate or clearly erroneous charge, or where the law requires. If the Practice stops offering health care services for any reason, we will refund any membership fees paid in advance for the period after services are no longer available.

Your privacy

We do not bill insurance and are not a HIPAA covered entity. We still treat your information as confidential, use reasonable safeguards, and do not sell it. You agree we may contact you by phone, text, email, patient portal, and telehealth for your care and account. Do not use these channels for emergencies.

Florida law

This agreement is governed by Florida law. Exclusive venue: Hillsborough County, Florida.

Electronic signature and consent

Electronic signatures are legally binding in Florida. By typing your name and checking the boxes below, you agree your electronic signature is the legal equivalent of your handwritten signature, and to receive and sign this agreement electronically. You may request a paper copy. We record the date, time, IP address, and terms version of your signature.

Precision Care Membership is not health insurance. Membership covers services included in the program and does not include hospitalizations, specialist care, imaging, emergency services, medications, or services not specifically listed.

The membership covers primary care services delivered within the provider's licensed scope of practice in Florida. It is a self-pay membership and is not a substitute for emergency care or for specialist care outside the practice.

What is direct primary care?

Direct primary care, or DPC, is a membership model for your everyday healthcare. You pay one flat monthly price for unhurried access to your provider, with no insurance company in the middle. It covers your routine primary care, from checkups and sick visits to chronic care and direct communication with your provider, so you always have a medical home you can reach. Precision is nurse-owned and cash-pay, which is what keeps your visits unrushed and your price predictable.

How is direct primary care different from using insurance?

With traditional insurance-based care, your visit is billed through your insurance company, which sets your copays, deductibles, and often how long your appointment runs. Direct primary care takes the insurance company out of your everyday visits. You pay Precision directly, one flat monthly price, with no copays, deductibles, or surprise bills for the services your membership includes. It is not insurance and does not replace it. It is a simpler, more direct way to get your routine primary care.

Do I still need health insurance if I have a membership?

Yes. The Precision Care Membership is not health insurance, and it is not a substitute for it. Your membership covers the everyday primary care services listed in the program. You should keep your own health insurance for the things the membership does not cover, such as hospital stays, specialist care, imaging, emergencies, and medications. Many members use their insurance for those larger needs and use the membership for their day-to-day care.

Is direct primary care the same as concierge medicine?

They share the same goal of closer, more direct access to your provider, but they are not the same. Concierge care is often a higher-cost model built around a large annual retainer. Direct primary care is built to be affordable and transparent: one flat monthly membership, no insurance billing for your included visits, and a price you can see up front. Precision is a direct primary care membership, priced for everyday families.

Is a direct primary care membership worth it?

It depends on how you use your healthcare. If you value reaching your provider directly, unhurried visits, same or next-day access when available, and a flat, predictable price with no copays or surprise bills, a membership can be a strong fit, especially if you manage an ongoing condition or simply want a real medical home. At $79/mo or $849/yr, it is designed to make consistent primary care affordable. If you would rather pay as you go, you can also book individual visits at a flat published price without joining.

You choose the plan that fits you. The monthly plan is $79/mo with a three-month initial commitment, then it continues month to month. The annual plan is a single prepaid payment of $849 that covers 12 months, saves about $100 versus paying monthly, and does not auto-renew. You simply choose to renew when the year is up.

Who provides my care at Precision?

Precision is nurse-owned and led by two experienced clinicians. Heather Ledford, RN is the founder, a registered nurse with more than 20 years of clinical experience. Sandy George, APRN-BC is a board-certified, Florida autonomous nurse practitioner with more than 30 years in healthcare, licensed to evaluate, diagnose, treat, and prescribe independently. Together they bring more than 50 years of combined experience, and your care plan is decided with you, in the room.
