Wellness and prescription disclaimer

Prescription therapies require a medical evaluation by a licensed provider before any prescription is written. These statements have not been evaluated by the Food and Drug Administration. Products and services offered by Precision are not intended to diagnose, treat, cure, or prevent any disease. Individual results vary and are not guaranteed.

Booking and payment

You can request appointments through this site. Precision is a direct, cash-pay practice and does not bill health insurance, so you are responsible for the cost of the services you choose. Payments are processed securely through our third-party payment processor, and we do not store your full card number on this site. Some services, including prescription therapies, begin with a free medical evaluation before any treatment or prescription.

Appointments are by appointment and subject to availability. If you need to cancel or reschedule, please let us know in advance. Cancellation and no-show expectations are handled in line with our clinic policy, which we will share with you when you book.

Membership

The Precision Care Membership is a direct primary care arrangement governed by a separate Direct Primary Care Membership Agreement that you review and sign when you enroll, under Florida's direct primary care law (Fla. Stat. 624.27). That signed agreement, not these Terms of Use, sets out the full terms of your membership, including what is included, billing, renewal, and cancellation. If anything in these Terms appears to conflict with your signed Membership Agreement regarding your membership, the signed Membership Agreement controls.

As stated in that agreement, the Precision Care Membership is not health insurance.

Intellectual property

The content on this site, including text, graphics, images, logos, the Precision name and brand, and the overall design, is owned by Precision Total Health & Wellness Clinic, LLC or its licensors and is protected by intellectual property laws. You may view and use the site for your own personal, non-commercial use. You may not copy, reproduce, republish, or distribute our content without our written permission.

Acceptable use

When you use this site, you agree not to:

• Use the site for any unlawful purpose or in violation of these terms
• Attempt to gain unauthorized access to any part of the site, our systems, or other users' information
• Interfere with or disrupt the site, its security, or its normal operation
• Use automated tools to scrape, harvest, or collect information from the site without our permission
• Submit false information or impersonate another person

Governing law

These Terms of Use are governed by the laws of the State of Florida, without regard to its conflict-of-law rules. Any dispute relating to these terms or the site will be handled in the state or federal courts located in Hillsborough County, Florida, and you agree to that venue.

Changes to these terms

We may update these Terms of Use from time to time. When we do, we will revise the effective date at the top of this page. By continuing to use the site after changes are posted, you accept the updated terms.
